Copy of W-2 forms for the previous two years (if commissioned or paid by 1099, copy of last two years complete tax returns)

Copy of last two years tax returns (personal and business)

Assets

Copy of statements for previous two months for bank and brokerage accounts
Copy of statement for retirement accounts

Liabilities

Copy of statements for all existing mortgage loans
Name and address for landlords for the previous two years

Miscellaneous

Copy of driver's license and social security card
Copy of lease agreements on all rental properties
Veterans! Copy of DD 214 and Certificate of Eligibility
Copy of fully executed divorce decree (if applicable)
